Char Name changed

hiyas, just logged in on my champ and his name was changed from Kyleman to Klyemann, the quickbars were reset also. i had a char on alb with that name which i know i deleted to use this name on a hib char. That alb char was back with the name Kyleman so deleted him again and made some char on that slot. bought a change name stone and changed it, lost the 10% rp, but those 6k rp dont matter. My question is, has this happened before and could it happen again? PS. i deleted the Alb with the Kyleman name a couple of months ago
