What items are being used to achieve this?

Cruise the itemdb and look. Can't sort by level, but you can do by class and get it down to a few pages.

For example, Deepscale Armor from Koalinth - but I think you need level30 for that one.

I heard you can drop rogs with those type of proc and with a low level condition. Might be this type of item.
